$20,000 Sign-On Bonus Advanced Practice Provider (NP or PA) Hospitalist & Rural Health Clinic Provider Providing care for all ages
Position Overview:
Sparta Community Hospital is seeking a qualified Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to support both Hospitalist services and Rural Health Clinic (RHC) operations. This position provides comprehensive patient care across the lifespan , while also supporting physicians through real-time clinical documentation (scribe responsibilities) . The provider practices collaboratively with physicians and other healthcare professionals in a variety of care settings.Responsibilities:
Clinical Care (RHC & Extended Care Settings) Provide high-quality, patient-centered care in collaboration with a physician across multiple settings, including the Rural Health Clinic (RHC), Long-Term Care (LTC), hospital, and patient residences.Responsibilities include:
Perform patient assessments, including: Comprehensive health histories Physical examinations Develop and manage individualized treatment plans, including: Prescribing medications Ordering and interpreting diagnostic tests Coordinating specialty referrals Provide preventive care and routine health maintenance Conduct pre-surgical history and physical exams and medical clearance evaluations Deliver Transitional Care Management (TCM), including: Initial post-discharge calls Follow-up visits within required timeframes Participate in telehealth visits and home health support as needed Perform Long-Term Care (LTC) visits and applicable procedures (e.g., wellness visits, stress testing, as trained) Provide patient education on: Disease prevention Treatment plans Health maintenance and wellness Scribe & Documentation Support Provide real-time clinical documentation support to Hospitalist physicians to ensure accurate and efficient patient records.Responsibilities include:
Document patient encounters within the Electronic Health Record (EHR), including: History of Present Illness (HPI) Review of Systems (ROS) Medical, family, and social history Physical exam findings Provider assessments and medical decision-making Treatment plans and follow-up instructions Record laboratory, imaging, and diagnostic results Assist with order entry, referrals, and care coordination documentation Support discharge planning and transitions of care documentation Compliance & Professional Standards Maintain strict adherence to: HIPAA regulations Patient confidentiality and privacy standards Deliver care and documentation in accordance with: Approved clinical protocols State licensure requirements Organizational policies and procedures Participate in the development and annual review of clinical protocols in collaboration with supervising physicians Maintain current knowledge of evidence-based medical practicesQualifications:
Education Graduate of an accredited program: Master's degree required for Nurse Practitioners Physician Assistant (PA): Completion of an accredited Physician Assistant program (Master's degree preferred or required based on program standards) Completion of required continuing education, including pediatric education as applicable Licensure & Certification Nurse Practitioner (NP): Current Illinois Registered Nurse (RN) license Current Illinois Advanced Practice Registered Nurse (APRN) license National certification (ANCC or equivalent) or Physician Assistant (PA): Current Illinois Physician Assistant license National certification through NCCPA Experience Previous experience as an NP or PA preferred; new graduates may be considered Experience in primary care, hospital medicine, or rural health settings preferred Skills & Competencies Current CPR, ACLS, and PALS certifications Proficiency with Electronic Health Record (EHR) systems Working knowledge of: CPT E/M coding HCC and ICD-10 coding principles Strong clinical judgment and critical thinking skills Excellent communication and interpersonal skills Ability to work collaboratively within a multidisciplinary team Work Environment & Schedule Schedule based on patient care needs across hospital, clinic, and community settings May include: Evenings Weekends Holidays Participation in on-call rotation with required response time Flexibility to work extended hours based on patient volume Work Relationships & Supervision Collaborates closely with physicians, nursing staff, and interdisciplinary team members Clinical care provided under established protocols and physician collaboration Documentation support (scribe role) performed under Hospitalist physician or Medical Director supervision Why Join Sparta Community Hospital Opportunity to practice across diverse care settings Collaborative, team-oriented environment Meaningful impact on rural community health Support for professional growth and clinical developmentSimilar remote jobs
