In this role, you'll provide direct patient care, indirect care, and inbox support within an integrated, team-based care model. You'll collaborate closely with behavioral health clinicians, clinical pharmacy, nursing staff, and an experienced exception support team, allowing you to focus on delivering thoughtful, high-quality care while practicing at the top of your license. Position Details:
Full time, 0.9 FTE 4-day schedule within a Monday to Friday workweek Team based, patient centered medical home model Integrated behavioral health, pharmacy, and nursing support Strong operational and inbox support to reduce administrative burden
Qualifications & Requirements:
Nurse Practitioner or Physician Assistant with 2+ year of post graduate clinical experience in a primary care setting (preferred) Interest in caring for patients of all ages, including pediatrics Collaborative, patient focused, and aligned with Providence's mission and values
