Job Summary:

Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply. Responsibilities Located in the Rogue Valley, White City allows easy access to Metro-area of Medford while remaining engrossed with nature. White City is an unincorporated community in Jackson County, Oregon, in the Rogue Valley. It is about nine miles north of Medford and 34 miles north of the California border. Enjoy the scenic drives to either Crater Lake, Oregon's only National Park, and to the mountain resort Lake of the Woods. Take a family-friendly hike to a waterfall or ride the only section of the Pacific Crest Trail that's open to mountain bikers. The Nurse Practitioner (NP) practices in Primary Care Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management. The NP practices to the full extent of their education, training, and certification. Core functions are outlined in the clinical privileges that correspond with this position. The NP has prescriptive authority for a panel of patients or program as indicated by their position. Functions or Scope of Assigned Duties include but are not limited to the following: The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients. Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members. Orders and interprets labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiates referrals and consults to specialty services. Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence.
