Job Description

1099 Position - RVU compensation module - Part Time NextGen Post Acute Care is a multi-specialty practice group specializing in pulmonary, infectious disease, cardiology, and post-acute care. We currently have over 100 providers delivering specialized care to patients in skilled nursing facilities across Florida, Ohio, Tennessee, and now in Pennsylvania and Texas. A core aspect of our program is reducing hospital readmissions, ensuring responsible stewardship, and optimizing patient care within these facilities. Our Corporate Headquarters is in Hallandale Beach Florida.

We're seeking a compassionate, experienced APRN to join our expanding team, providing patient-centered care across skilled nursing and long-term rehab settings. You'll diagnose, treat, manage chronic and acute conditions, and collaborate with interdisciplinary teams across specialties including pulmonary, infectious disease, cardiovascular, pain management, psychiatry, and rehabilitation.

Brady, TX Key Responsibilities:
Provide direct patient care in SNF/rehab/LTC settings Perform assessments, physical exams, and develop treatment plans Diagnose and manage acute and chronic conditions Prescribe medications and follow clinical best practices Document all care accurately in EMR/EHR Conduct admissions and discharges Collaborate with physicians, specialists, and facility staff Support preventive care and educate patients/families Contribute to reducing hospital readmissions Maintain adherence to infection control and regulatory standards Qualifications Active TX APRN license DEA (preferred), active
NPI BLS & ACLS
certifications Minimum 1 years nursing experience (SNF/post-acute preferred) Strong assessment, communication, and independent practice skills Experience in geriatrics, post-acute care, and EMR/EHR systems Schedule Part Time Benefits Competitive pay structure Documentation tool (Moxie Link) & census planning support Ongoing education and clinical support
Pay:
$30,000.00 - $60,000.00 per year
