Here, you're not just taking a job; you're joining a team where you belong. As a Home Health RN Case Manager, you will coordinate and deliver individualized care to patients in their homes. You'll work independently while collaborating with a supportive interdisciplinary team, helping patients recover, maintain independence, and achieve their health goals.

Your role is essential in helping patients feel safe, supported, and confident—right where they live. Develop and manage personalized care plans based on each patient's unique needs Perform comprehensive assessments and ongoing evaluations Provide skilled nursing care, treatments, and patient education Coordinate care with physicians, therapists, and other healthcare professionals Support patients and families in understanding diagnoses, medications, and care plans Monitor progress and adjust care to ensure the best outcomes Document care accurately and meet all regulatory standards Build meaningful, trusting relationships with patients and their families Help bring care, comfort, and compassion home to those who need it most. JOB REQUIREMENTS
RN or BSN degree from an accredited College of Nursing, Nurse Licensure as a registered nurse in Iowa. One year clinical experience preferred; or . Home care or community health nursing experience preferred. Car, driver's license and adequate insurance as outlined in the personnel policies. All offers of employment are conditioned upon the result of a criminal background check conducted by the Iowa Division of Criminal Investigation, as required by law and a negative finding on the Office of Inspector General's Excluded Provider List.
