$25.4982-$34.3728 Job Summary In collaboration with, and under the guidance of, licensed Registered Nurses assists with patient care and general duties on the assigned nursing unit. Assignments consist of 7-15 patients based on patient need and acuity. Ensures program and scheduled appointment times are met. Transports patients to and from therapies and special appointments as necessary. Assists with non-nursing functions on unit as needed. Essential Functions Responsible for initiating and completing direct patient care to an assigned group of patients, i.e., giving bed baths, grooming, feeding, showering, transferring patient from bed to wheelchair or toilet, preparing and transporting patients to and from X-ray, obtaining vital signs. Reports abnormal vital sign readings or other changes in patient's status to the primary RN assigned to the patient before moving on to the next patient or task. If primary RN is on break, reports findings to other appropriate RN (Charge RN, Nurse Manager, etc.) Performs functional duties, i.e., maintains a clean, orderly and safe environment for patient, does simple cleaning of showers, pantry, etc., puts call light within patient's reach, wipes up spills from floor when indicated, makes beds, etc. Responsible for attending to call bells within the timeframe defined by the nursing department. Supports the care team by responding to bells within own assigned group of patients and/or those of other team members as needed. Escorts patients to outside medical appointments as needed. Collaborates with all members of the care team to ensure patient needs are promptly attended to; operates under the license and guidance of Registered Nurses on the unit. Appropriately communicates challenges that arise (ex. scheduling conflicts, needing help with prioritization of tasks, etc.) utilizing supervisors on duty to provide clarification (ex. Charge RN, Nursing Supervisor, Nurse Manager, etc.). Practices good communication during hand-offs (ex. at break-times, shift change, when leaving the unit) to inform care team and ensure continuity of care and patient safety. Uses teamwork and collaboration with nurses and other nursing assistants to provide care to patients on the unit. Assists with admission (including orientation to rehabilitation) and discharges of patients, i.e., documentation of clothing and valuables and/or packing and checking them, dispensing supplies, etc. Able to do non-technical nursing procedures i.e., weighing patient, applying prosthesis, restraints, application of anti-embolism devices. Checks on assigned patients frequently, practicing hourly rounding; reports any changes in patient's condition, reports relevant patient requests, needs for supplies, equipment, etc., to nurse. Assists with the ambulation of patients on unit. Observes patients during respiratory treatments and/or other procedures; assists clinician to ensure patient safety. Assists with the orientation of nursing aides, students and volunteers. Assists patients to and from therapies as well as on and off patient elevators. Assists with special nursing procedures, i.e. simple wound care, straight cath, colostomy care, blood pressure, catheterizations, sterile dressings, bowel routines, etc. May assist with non-nursing functions on units to include clerical and telephone responsibilities. May assist in the operating of the elevator to include adhering to procedures relating to elevators (code 4, ambulance and safety during elevator use/operation), may assist with transportation of patients to and from therapies. Brings wheelchairs to/from wheelchair shop as needed. Demonstrates the ability to utilize knowledge of age related changes within a patient population to offer appropriate care to the population served (ex. adolescent, young adult, adult & geriatric patients). Assist with keeping unit clean, i.e. utility room and discharged patient belongings closet. Practices Standard Precautions. Follows regulations, policies and guidelines on the prevention of infections; does not eat or drink in patient care areas Personally adheres to BurkeC.A.R.E.S.

Qualifications High School Required or GED Required 1-3 years Minimum 1 year clinical experience Preferred Basic computer skills preferred. Ability to prioritize effectively when presented with competing demands; able to multi task effectively. Ability to execute tasks independently and as part of a collaborative team. Interacts with the rehabilitation team, patients, families and visitors. Frequent lifting, pushing, pulling and transferring of patients. Continuous standing, bending and walking. Walking and pushing wheelchairs, stretchers. Physical and manual dexterity, visual and auditory acuity. Good communication skills, diplomatic and tactful Ability to learn clerical tasks; and use of technology (e.g., computers, phones) in execution of tasks BLS - American Heart Association Preferred NYS Certified Nurse Aide (CNA) Preferred Work Schedule - 13 shifts per 4-week scheduling period, 7:00p-7:00a.
