Full‑Time Position Summary The Nutritionist / Registered Dietitian Nutritionist (RDN) provides comprehensive nutrition assessment, counseling, and medical nutrition therapy to primary care adult and pediatric patients as well as individuals enrolled in the Ryan White
HIV/AIDS
Program within a Federally Qualified Health Center (FQHC). Working as part of an interdisciplinary, team‑based care model, the RDN supports disease prevention and management, HIV care, maternal and child health, and overall wellness for underserved populations. This role emphasizes culturally responsive, patient‑centered care and addresses nutrition‑related social determinants of health to improve access, engagement, and health outcomes. Essential Duties and Responsibilities Clinical Nutrition Services (Primary Care - Adult & Pediatric) Conduct comprehensive nutrition assessments using anthropometric, biochemical, clinical, dietary, and socioeconomic data Develop and implement individualized nutrition care plans based on evidence‑based guidelines Provide medical nutrition therapy (MNT) for conditions including diabetes, hypertension, obesity, cardiovascular disease, pediatric growth concerns, prenatal and postpartum nutrition, and general wellness Deliver individual and group nutrition education to patients and families using culturally and linguistically appropriate approaches Collaborate with primary care providers, nurses, behavioral health clinicians, and care coordinators as part of the interdisciplinary care team Ryan White

Program Services Provide nutrition counseling and MNT to Ryan White eligible clients to support treatment adherence, viral suppression, and overall health Address HIV‑related nutrition needs, including weight changes, metabolic complications, and medication‑related side effects Collaborate with Ryan White medical case managers and support service staff to address nutrition‑related barriers to care Screen for food insecurity and connect Ryan White clients to internal and community resources Document services in compliance with Ryan White program requirements, including accurate and timely entry into the electronic health record and applicable reporting systems Care Coordination, Documentation, and Quality Document all patient encounters accurately and timely in the electronic health record (EHR) Participate in care conferences, huddles, and quality improvement initiatives Support health center performance measures related to chronic disease management, preventive care, and retention in care Assist with program reporting and compliance related to HRSA, Ryan White, and other applicable funding sources Required Qualifications Bachelor s or Master s degree in Nutrition, Dietetics, or a related field from an accredited institution Registered Dietitian Nutritionist (RDN) credential required Current state licensure or eligibility for licensure, as applicable Experience providing nutrition services in a clinical or community health setting Experience working with underserved populations preferred Knowledge, Skills, and Abilities Strong knowledge of medical nutrition therapy, chronic disease management, pediatric and adult nutrition, and HIV‑related nutrition needs Ability to provide trauma‑informed, culturally competent, and patient‑centered care Excellent communication, counseling, and documentation skills Proficiency with electronic health records and basic computer applications Ability to work effectively in a fast‑paced, team‑based FQHC environment Commitment to health equity and the mission of community health centers Preferred Qualifications Experience working within the Ryan White

Program Certified Diabetes Care and Education Specialist (CDCES) or eligibility Bilingual (Spanish or other languages commonly spoken in the service area) Experience addressing food insecurity and social determinants of health
