Working towards a Bachelor's Degree required. Bachelor's Degree Preferred. Minimum 1-year experience with DD population preferred. Key Responsibilities include, but not limited to: Manage caseload of approximately 25 consumers participating in Life Options. Develop and maintain consumer schedule based upon consumer choice as identified in ISP, through valued outcomes, team, and family/guardian input. Collect and review data and document progress for monthly notes. Develop IPP and valued outcomes/informal outcomes for all consumers. Conduct meetings with groups of consumers to develop ideas for future learning modules. Provide guidance and counseling to consumers in relation to learning modules/and the consumer's goals and administer pre-tests for selected learning modules. Refer consumers for clinical evaluations as needed. Conduct Annual and Semi-Annual Case Conference meetings. Coordinate effective delivery of services by maintaining contact with family/guardian/residence/MSC. Ensure needs of consumers are met including but not limited to their safety and well-being. Ensure compliance with

OPWDD/OPTS

regulations and Life Options' Policy & Procedures, to include but not limited to training on behavioral plans/strategies & protocols. Assist with ADL needs of consumers including, but not limited to, feeding, transferring, lifting, toileting, dressing (coats), etc. as per consumer's protocols.

Physical Demands:

Must be able to bend, lift and push more than 100 pounds. Must be able to stand and walk for extended periods of time. Provide hand-over-hand assistance in classroom when necessary. Join our Team and Make a Difference!
