Our skilled nursing facility is transitioning to an in-house therapy program , and we're building a collaborative, clinician-led team from the ground up. • no loss of hours due to low census What You'll Do Provide skilled, evidence-based therapy services to short-term and long-term residents Collaborate with an interdisciplinary team to create and implement individualized care plans Participate in program development, quality initiatives, and best-practice standards Document care accurately and timely in compliance with regulations What We're Looking For Current state license required for your discipline SNF experience preferred, but new grads welcome Passion for patient-centered care and teamwork Strong communication and clinical reasoning skills
