Job Summary:
We are seeking a highly motivated and compassionate Occupational Therapist to join our team at Palms Care Center, a leading provider of Skilled Nursing Services in Lauderdale Lakes, Florida. About the
Role:
As an Occupational Therapist with Palms Care Center, you will be an integral part of our interdisciplinary team, working closely with our residents, families, and healthcare professionals to develop and implement personalized care plans that promote independence, safety, and well-being. Your expertise and guidance will help our residents regain their strength, mobility, and confidence, enabling them to return to their daily routines and live life to the fullest.
Responsibilities:
Conduct comprehensive assessments of residents' physical and cognitive abilities, identifying areas of strength and weakness, and developing targeted interventions to address these needs Design and implement individualized occupational therapy plans to promote independence, safety, and well-being, incorporating evidence-based practices and innovative techniques Collaborate with our multidisciplinary team, including physicians, nurses, and rehabilitation therapists, to ensure seamless care delivery and optimal outcomes Develop and maintain strong relationships with residents, families, and caregivers, providing education, support, and guidance to empower them to take an active role in their care Keep accurate and detailed records of resident progress, documenting all assessments, treatments, and outcomes in accordance with regulatory standards and organizational policies Participate in quality improvement initiatives, staying up-to-date with best practices and contributing to the ongoing development of our clinical programs
