  • Perform evaluations of
ADL/IADL
function and home safety
  • Develop individualized treatment plans
  • Provide therapy to improve independence and safety
  • Educate patients and caregivers
  • Recommend adaptive equipment and home modifications
  • Collaborate with interdisciplinary care team
  • Maintain timely and accurate documentation Qualifications
  • Active California OT license
  • Home Health or geriatric experience preferred, not required
  • Strong communication and clinical skills
