Join our dynamic healthcare team as an Occupational Therapist, where your expertise will empower individuals to achieve independence and improve their quality of life. In this vital role, you will assess patient needs, develop personalized care plans, and implement therapeutic interventions across diverse settings. Whether working with children in schools, adults in outpatient clinics, or seniors in skilled nursing facilities, you'll make a meaningful difference through compassionate, evidence-based care. Responsibilities Conduct comprehensive assessments of patients' physical, cognitive, and emotional functioning to identify their unique needs and goals. Develop and implement tailored treatment plans that promote functional independence and enhance daily living activities. Assist patients with Activities of Daily Living (ADLs), including dressing, grooming, feeding, and mobility techniques. Coordinate discharge planning by collaborating with multidisciplinary teams to ensure smooth transitions from hospital to home or other care settings. Work with individuals with autism, disabilities, geriatrics, pediatrics, or those recovering from acute care or ICU stays to optimize their functional outcomes. Provide therapy services in various environments such as inpatient units, outpatient clinics, home health visits, schools, or skilled nursing facilities. Document patient progress accurately and maintain detailed records aligned with healthcare standards and regulations. Engage in case management activities to monitor patient progress and adjust treatment plans as needed for optimal results. Skills Strong knowledge of physiology, anatomy, and medical terminology relevant to occupational therapy practice. Proven experience assisting with ADLs and developing effective care plans for diverse populations including pediatrics, geriatrics, and individuals with disabilities. Familiarity with discharge planning processes and case management strategies across inpatient, outpatient, home health, and post-acute care settings. Experience working with individuals with autism or developmental disabilities in clinical or school environments. Ability to apply occupational therapy techniques in acute care settings such as ICU or hospital units. Knowledge of early intervention practices and occupational health principles for workplace wellness programs. Excellent patient care skills combined with strong communication abilities to collaborate effectively within multidisciplinary teams.
