Service Coordinator SALARY:
$5,261.00/ Month -Plus Excellent Benefits CLOSING:

CDDPGENERAL DESCRIPTION OF POSITION
The Services Coordinator coordinates and provides services to people with intellectual/developmental disabilities through case management and coordination of services. Service coordination involves identifying, assessing, collaborating on, and monitoring the delivery of services and support required by individuals with intellectual/developmental disabilities and their families to ensure health and safety, personal choice, and community inclusion, as outlined in OAR 411.320, 411.415, and 411.425. Services are provided in and out of the office environment. This position relies heavily on timelines, written service plans, and the ability to multitask.SUPERVISORY RELATIONSHIPS
Supervision received: works under the supervision of the Intellectual/Developmental Disabilities Supervisor and the Program Manager/Director.PRINCIPLE DUTIES OF POSITION
Provide case management services to eligible individuals, which include but are not limited to: Interview people to obtain needed information through reciprocal communication. Facilitate meetings with individuals, their identified teams, professionals, and contractors. Identify the physical, social, medical, employment, and other needs based on gathered information. Complete and update annual plans, risk assessments, functional needs assessments, and other necessary documentation required for needed services within strict timelines. Authorize and monitor services. Make referrals to available services and supports and assist individuals in accessing services and resources they are eligible for. Coordinate with public and private agencies locally and statewide. Participate in assessing community needs and planning activities for developing needed services. Attend meetings and training to provide and receive input as requested. Report and track serious events according to state requirements. Access and utilize differing web-based programs. Perform other related duties as assigned. This document in no way implies that these are the only duties to be performed by the employee occupying this position.MINIMUM REQUIREMENTS FOR THE SERVICE COORDINATOR POSITION
Must know the public service system for developmental disabilities services in Oregon and at least: A bachelor's degree in behavioral or social science or a closely related field; or A bachelor's degree in any field and one year of human service-related experience, such as work assisting individuals and groups with issues such as economic disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing; or An associate degree in behavior science, social science, or a closely related field and two years of human services-related experience, such as work assisting individuals and groups with issues such as economic disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies or housing; or Three years of experience working specifically with individuals/families identified as having significant support needs or an agency/provider that provides services directly to individuals with significant support needs. And Valid Driver's license. Ability to pass a pre-employment drug screening. Ability to pass local and state background checks.COMPETENCIES REQUIRED TO BE DEMONSTRATED ON AN ONGOING BASIS

Able to demonstrate the competencies required by OAR and compliance with DD Program, Umatilla County policy, and procedures. Able to travel to meetings, training, and conferences within Umatilla County and the State of Oregon. Must participate in 20 hours per year of State of Oregon-sponsored training or other training in the areas of intellectual or developmental disabilities. Must demonstrate the ability to act on personal initiative using sound judgment.
