Job Description

Outpatient Counselor 2801 C Court, Ashtabula, OH 44004 Full-time Full-time Lake Area Recovery Center is seeking a motivated and compassionate Outpatient Counselor to join our growing behavioral health team. This position provides substance use and co-occurring mental health counseling services to individuals involved in treatment through individual counseling, group counseling, case coordination, and treatment planning. The ideal candidate is team-oriented, organized, professional, and passionate about helping individuals achieve long-term recovery and stability. Responsibilities Conduct assessments, screenings, and intake interviews Develop individualized treatment plans Provide individual, group, and family counseling services when needed Maintain accurate and timely clinical documentation Coordinate care with courts, probation officers, hospitals, physicians, and community agencies Assist clients with referrals for housing, employment, medical care, and community resources Monitor client progress and participate in treatment team meetings Maintain compliance with agency, CARF, Medicaid, and state documentation standards Facilitate psychoeducational and recovery-focused groups Participate in staff meetings, trainings, and clinical supervision Qualifications
CDCA, LCDC II, LCDC
