Outreach Case Manager

Provide case management to adults with substance use disorders (SUD) who are experiencing homelessness and/or are at risk of experiencing homelessness. Responsible for outreach and engagement strategies to unsheltered (including encampments) and sheltered populations to increase access to, and participation and retention in, case management, treatment, and recovery support services. Implement referral pathways that engage all demographic groups of the community. Documents services appropriately and timely, using agency approved formats, that clearly describe the need for the service, the intervention provided, the date, actual time and setting of the service, and the individual's response to the service. Help individuals to create their own recovery plans and develop their own recovery pathways while also helping the person focus on long term recovery, health, and wellness. Engage the patrons with warm handoff to community partner agencies. Submit statistical information and program reports to Executive Director and Director of Case Management. Qualifications for

Position:

Must pass background check and must not be on any "do not hire" list with the Department of Mental Health or the Missouri Division of Family Services.

Skills and Knowledge Required:

This position must build relationships with individuals seeking recovery by being grounded in trust, focused on providing the individual with tools, resources, and individualized support to achieve long-term recovery. Maintains confidentiality and treat everyone with dignity and respect. Able to set and observe appropriate personal limits and boundaries, especially with respect to shelter guests. Conducts services in a culturally competent manner.

Amount of Travel/Other Special Conditions:

Extensive travel and field work within the local region may be required. Must have a valid driver's license and daily access to an automobile that is properly licensed and insured. CPR and First Aid certificates preferred but not required.

Supervisory Relationships:

This position reports to the Outreach Case Manager and has no supervisory requirements
