Job Description

Job Summary We are seeking a dedicated and caring Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide exceptional care to our residents in a supportive and team-oriented environment. You will play an essential role in delivering quality nursing care, promoting dignity, and helping residents achieve their highest level of well-being.
Essential Duties and Responsibilities:
Resident Care Provide direct nursing care to assigned residents following individualized care plans and physicians' orders. Administer medications and treatments accurately and timely in accordance with facility policies and state regulations. Observe residents and report significant changes in condition to the RN, physician, or appropriate supervisor. Assist with admissions, transfers, and discharges of residents. Perform and document ongoing assessments, interventions, and outcomes in the electronic health record (EHR). Supervision and Leadership Supervise, guide, and support nursing assistants to ensure quality resident care and adherence to care plans. Promote teamwork and open communication among staff to maintain a positive, respectful work environment. Participate in staff education, training, and performance feedback as assigned. Documentation and Communication Accurately chart all medications, treatments, and nursing interventions. Communicate effectively with residents, families, physicians, and interdisciplinary team members. Participate in resident care conferences and quality improvement activities. Safety and Compliance Follow infection control, safety, and emergency procedures at all times. Maintain confidentiality of resident information in accordance with HIPAA regulations. Ensure all nursing practices comply with state and federal long-term care requirements.
Qualifications:
Current Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the State of Minnesota. Current CPR/BLS certification. Prior experience in long-term care or geriatric nursing preferred, (but not required—new grads welcome!). Ability to communicate effectively and compassionately with residents, families, and staff. Strong attention to detail, organization, and professionalism. Ability to lift, move, and assist residents as required.
