We are seeking a compassionate and adaptable Palliative Care Admit/Flex Nurse (RN) to join our dedicated interdisciplinary team serving patients across Maui County. This role is ideal for an experienced RN who thrives in a flexible environment, enjoys building meaningful patient relationships, and is passionate about providing exceptional palliative and hospice care in homes, hospitals, facilities, and community settings. What You'll Do As a Palliative Care Admit/Flex Nurse, you will: Conduct palliative care and hospice admissions and comprehensive nursing assessments Coordinate smooth transitions from hospital to home, facility, or Hospice Hale Provide compassionate direct patient care and symptom management Educate and support patients, caregivers, and families Collaborate closely with physicians, social workers, chaplains, aides, and the interdisciplinary team Communicate with referral sources and community partners Complete accurate clinical documentation and care planning Help foster Hospice Maui's reputation for excellence and compassionate care throughout the community Qualifications Current Hawaii RN license in good standing Current CPR certification Valid driver's license and reliable transportation Minimum two years of nursing experience preferred Hospice, palliative care, home health, or hospital experience preferred Strong communication, critical thinking, and organizational skills Comfortable working independently while collaborating with a supportive team Commitment to hospice and palliative care philosophy Why Join Hospice Maui?
