Job Description:
The Part-Time Case Manager is responsible for working with clients to develop a personalized case management plan that addresses their barriers to self-sufficiency. Responsible professional work performed under the direct supervision of the Community Services Coordinator. This is a part-time daily position, 25 hours per week.
Key Responsibilities:
Interpret programs operated and provide outreach to all community agencies and the general public. Develop and maintain cooperative working relationship with all other public and private social service organizations. Act as a primary source of communication regarding the needs of low-income citizens to management to ensure effective use of available program funds. Prepare reports and required documents; conduct surveys, maintain accurate records and recommend changes in program direction as needed. Work with clients to develop individualized case management plans, to include ongoing follow ups and supportive services such as community referrals, employment services, etc. Adheres to strict boundaries and professional ethics. Assist client with program application, act as liaison between client and client service providers. Responsible for all data entry required to capture and track client data and demographics; maintain client logs and fund balances. Attend program-related community coalition and committee meetings and trainings as assigned by supervisor. During designated months of the year, LIHEAP intake appointments will be incorporated into daily duties. Responsibilities include assisting clients with LIHEAP applications, collecting and reviewing required documentation, verifying eligibility information, coordinating services with utility vendors, and educating clients on energy conservation practices and strategies to reduce long-term energy costs. Possess reliable transportation and availability to travel, including overnight when necessary. Must be a US citizen; have a Permanent Resident Card, or an alien Registration Receipt Card (Form I-551) Must have and maintain a clean driving record (NOT
TO EXCEED 6 PTS
) Must have and maintain a criminal history absent of convictions for violent crimes or for any crime against children. High school diploma or equivalent required. Experience working in a community based organization utilizing resources to assist low income highly desired Associate's or Bachelor's degree in social work or closely related field preferred
