Scrubs required Position Overview We are seeking a reliable, patient-focused Patient Care Management (PCM) Technician to work on-site in a busy dermatology clinic. This role is similar to Chronic Care Management (CCM) and is designed to improve communication, continuity of care, and timely provider updates. The PCM Technician will enroll patients in person, assist with care plan follow-ups, support our remote Care Center call agents, and ensure all patient interactions are accurately documented in the clinic's EHR ( eClinicalWorks ). This role plays a critical part in ensuring providers receive same-day patient updates , allowing for more responsive dermatologic care. Key Responsibilities Enroll eligible patients in the PCM program in person at the clinic Educate patients on their care plan and answer basic program-related questions Collaborate with Care Center call agents by assisting with patient outreach and follow-up questions Conduct patient check-ins related to care plans and provider-directed questions Accurately document all patient interactions, responses, and updates in eClinicalWorks (EHR) Communicate patient responses and clinical updates to the provider daily Act as a liaison between patients, providers, and care coordination teams Ensure timely escalation of patient concerns or urgent findings to the clinical team Maintain HIPAA compliance and professionalism at all times Qualifications Medical office or healthcare experience preferred Familiarity with medical terminology required Experience with EHR systems preferred (eClinicalWorks experience a plus, not required) Strong communication and interpersonal skills Comfortable working directly with patients in a clinical setting Detail-oriented with strong documentation skills Ability to work independently while collaborating with remote teams Professional appearance and comfort wearing scrubs
