Blended Rate Key Responsibilities:

Perform medication reconciliation , including discharge medication lists Obtain accurate prior-to-admission medication histories from patients, families, and pharmacies Provide discharge counseling to patients and caregivers Ensure safe and effective medication use across transitions of care

Clinical Expertise Required:

Antimicrobial Stewardship Interpret culture results and provide therapy recommendations Strong knowledge of infections including UTI, pneumonia, and SSTIs Pharmacokinetics Dosing, monitoring, and adjustments based on patient-specific factors Anticoagulation Management Dosing and monitoring of anticoagulant therapies

Requirements:

Active OR Pharmacist License (REQUIRED) EPIC experience required Must have Hospital experience Strong clinical judgment and communication skills Ability to collaborate effectively with interdisciplinary teams #
