Responsibilities include:

Managing pharmacotherapy for diverse medical conditions Applying pharmacokinetic and renal dosing principles Monitoring therapeutic medication levels Handling drug shortages effectively Utilizing EPIC electronic medical records for patient care documentation Collaborating with healthcare teams to optimize medication use

Qualifications and Experience:

Minimum 2 years of recent inpatient or acute care pharmacy experience Licensed to practice pharmacy in Wisconsin (ACT-10 license accepted) Competence in therapeutic medication monitoring and pharmacotherapy management Experience with

EPIC EMR

system preferred

Schedule:

Rotating 5 shifts per week, 8 hours each Mix of day shifts (6:30 AM-3:00 PM) and evening shifts (2:30 PM-11:00 PM) Rotation includes working every third weekend
