Category Pharmacy - Clinical Pharmacist Job Type Direct Hire A hybrid clinical/staff pharmacist role is now available at a respected medical center in State College, PA. This position offers a blend of patient-centered clinical work and hands-on pharmacy staffing—allowing you to make a direct impact on medication management while supporting the pharmacy team. In this role, approximately 60% of your time will be devoted to clinical responsibilities focused on Population Health and Ambulatory Care. You will collaborate with patients to optimize medication regimens, particularly targeting chronic disease states such as diabetes. The remaining 40% involves traditional staffing duties within the pharmacy. There is strong potential for this position to evolve into a fully clinical role as the medical center expands its clinical services.

Responsibilities:

Manage and optimize medication therapies for ambulatory care patients, focusing on chronic disease management Provide clinical support and consultation to healthcare providers and patients Participate in staffing duties, including dispensing and inventory management Assist in the development and growth of clinical pharmacy programs Utilize Meditech for documentation and pharmacy workflow

Qualifications:

Pharmacist licensed to practice in Pennsylvania Experience in ambulatory care or inpatient pharmacy settings preferred Residency training highly desirable Strong clinical knowledge with a passion for patient care and population health Effective communication skills and team-oriented mindset
