THIS IS A PER DIEM / PRN POSITION
Job Summary Works under the supervision of the Oncology Pharmacy Manager, or Director of Pharmacy, to provide high quality pharmacy services to the Oncology Clinic. Supports the mission, vision and values of Skagit Regional Health, an integrated healthcare system. Responsible for comprehensive pharmaceutical care through order processing, clinical services, and patient education. Provides immediate supervision and guidance of the work activities of Pharmacy Technicians and retains responsibility for the work performed by Pharmacy Technicians and Interns. The Pharmacy department values teamwork, commitment to patient centered care, and clinical excellence. Pharmacists are responsible to verify the accuracy, quality, and quantity of Controlled Substances and Legend drugs that are transferred to Automated Dispensing Machine (ADM), Nurse, Provider, or patient. Pharmacists are responsible for clinical and distributive activities throughout Skagit Regional Health Oncology clinics, and participates in therapy of patients admitted for oncology therapy at Skagit Valley Hospital, or Cascade Valley Hospital. Pharmacists are responsible to maintain adequate drug, Legend and Controlled Substance, inventory available for patient care. Pharmacists may be assisted by Pharmacy Technicians, under their supervision, to order, manage, prepare, and distribute pharmaceutical inventory products in compliance with regulatory and SVH policies and procedures. Essential Functions Focuses on customer satisfaction and needs, through effective communication during all interactions, and consistent with SRH Service Standards and Code of Conduct. In the absence of the Oncology Pharmacy Manager, this position is responsible for the day to day operations of Oncology Pharmacy. Maintenance of medication records utilizing electronic health records, preparation and dispensing of intravenous drug therapies, monitoring of drug regiment appropriateness, prevention and monitoring of adverse drug effects, oversight of medication safety and security, provision of drug information to Skagit Regional Health Oncology patients and professional staff, performance of clinical drug interventions with medical record documentation. Provides effective, efficient and accurate pharmaceutical care that meets both department financial and customer service goals and objectives. Work is performed in accordance with protocols, internal guidelines/standards, and Washington State Board of Pharmacy regulatory requirements. Work involves the oversight of all aspects of Oncology Clinic pharmacy operations, including dispensing medications and intravenous solutions to patients for all types of chemotherapy and ancillary treatments; providing comprehensive pharmaceutical care to Oncology patients. Responsible for the provision of prospective and concurrent evaluation of pharmacotherapy with an emphasis on the patient and with the end point focused on outcomes such as treatment efficiency, morbidity, mortality and drug cost. Acts as a liaison to medical, nursing, pharmacy practitioners and other healthcare professionals. Develops clinical pharmacy standards related to oncology pharmacy according to the pharmaceutical care model and organizational policies/procedures for ensuring the quality of medication therapy. Performs all distributive functions of department; physician order interpretation, order entry into EMR, therapy monitoring, IV admixture functions, supervision of technician activity, monitoring delivery systems, computer operation and stock control. Compounds and dispenses prescribed medications, drugs, IV solutions, and other pharmaceuticals for patient care in compliance with regulatory and department policies and procedures. Reviews orders placed by authorized prescribers, to assure accuracy and safety. Inputs patient and medication data to maintain pharmacy files, charge and inventory data in an accurate and timely manner in compliance with SVH policies and procedures. Assists Pharmacy and Oncology leadership in support of department clinical functions including, but not limited: ADR, DUE, P&T, and on-going clinical staff education. Works collaboratively with all Pharmacy and ancillary staff to meet financial and customer service goals such as medication cost and inventory control, efficient workflow, insurance billing, and controlled substance controls. Contributes to the achievement of Quality Improvement and participates in QI activities. Ensures compliance with established standards, and all SRH policies and procedures. Complies with SVH Safety and Infection Control policies and procedures. Utilizes proper body mechanics and protective equipment (PPE). Reports and/or resolves all safety issues/incidents in a timely manner. Complete all mandatory learning requirements in a timely manner. Maintain current licensure and/or certification as required. Attend and actively participate in all department, team and/or committee meetings. Comply with all Skagit Regional Health, department and location specific policies and procedures. Performs other duties as assigned by Director, Oncology Manager, or Designee. Education Graduate from an accredited pharmacy college recognized by the American Council of Pharmaceutical Education (ACPE). Acute Care and Oncology residencies and / or Board Certification (BCPS and BCOP) are highly preferred. Experience/Training Two years of pharmacy experience is preferred. Prior Oncology Pharmacy experience is highly preferred. Advanced knowledge of applicable federal and state pharmacy laws and regulations is required. License/Certifications Must have an active Washington State pharmacy license held in good standing. BLS / CPR, or ability to obtain within probationary period. Other Skills Qualified to participate in federal health care programs, including but not limited to Medicare and Medicaid. Knowledge of laws governing drugs and pharmacy products is required. Self-motivated with the ability to complete work with minimum supervision under tight time frames. Must maintain honesty and integrity, and display high degree of professionalism, efficiency, and accuracy at all times. Demonstrated proficiency with multi-tasking, change acceptance, initiative, customer service, interpersonal relations and conflict resolution, decision making, problem solving, teamwork, verbal and written communication. Flexibility to work at other locations as necessary, and the ability to work a variety of schedules to include long hours, evenings. Working knowledge of Microsoft Office and pharmacy related software required, EPIC experience preferred. Physical Demands and Work Environment Must be able to tolerate long periods of standing and / or sitting, repetitive, bending, stooping, kneeling, lifting, stretching and reaching. Must be able to lift 30 pounds, without assistance.
