Position Responsibilities:

Perform in-home physical therapy evaluations and develop individualized treatment plans Provide high-quality therapy services to patients in their home environment Focus on improving strength, mobility, balance, safety, and functional independence Assess home environments and recommend modifications or equipment to improve patient safety Collaborate with physicians, nursing staff, caregivers, and interdisciplinary teams Educate patients and families on treatment plans, exercises, and long-term wellness strategies Complete timely and accurate documentation in compliance with home health standards

Qualifications:

Active Physical Therapist license Home Health, geriatric, or rehabilitative therapy experience preferred Strong clinical assessment and communication skills Ability to work independently and manage patient schedules effectively Positive, compassionate, and team-oriented attitude Passion for patient-centered care and improving lives at home
