We are seeking a Full-Time SNF Physical Therapist to join our team at Bear Creek Senior Living in Colorado Springs, CO. Your duties will include:
Providing direct patient care, evaluating functional needs and outcomes, and consulting with other specialists Reviewing patients' functional needs and adjusting care plans to ensure optimal patient outcomes Designing and implementing individualized plans of care based on thorough assessments of potential development, incorporating participation by significant others whenever appropriate Conducting therapy practice within defined standards of care Directing the assistance of support staff to achieve patients' functional goals Communicating effectively with other professional and support staff to achieve positive patient outcomes Practicing facility and departmental policies and procedures, including quality improvement, safety, environmental, and infection control standards Documenting evaluations, treatment goals, and plans; regularly updating documentation and maintaining prescriptions and signatures Enhancing professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops Qualifications Graduate of an American Physical Therapy Association (APTA) approved physical therapy program Current Physical Therapist license from the state of Colorado Board of Physical Therapy Examiners Knowledge and skills to care for the physical and developmental needs of the population age groups that will be receiving services At least one year of experience in a skilled nursing or rehabilitation setting preferred Ability to remain calm in emergency or crisis situations Strong interpersonal and communication skills How to Apply If you're ready to bring warmth and joy into the lives of our senior residents, we'd love to hear from you!
