Job Description

Position Summary The Physical Therapist is responsible for evaluating and treating individuals with physical limitations, mobility challenges, or developmental disabilities to improve movement, strength, balance, and overall physical functioning. This role focuses on developing individualized therapy plans that promote independence, prevent injury, and enhance quality of life. The Physical Therapist works collaboratively with interdisciplinary teams, caregivers, and program staff to support individuals in achieving their physical health and mobility goals. This position provides therapeutic interventions, monitors progress, and ensures services are delivered in accordance with professional standards and regulatory requirements. Key Responsibilities Assessment and Evaluation Conduct comprehensive physical therapy evaluations to assess mobility, strength, coordination, balance, and physical functioning. Review medical history and collaborate with healthcare providers and caregivers to understand individuals' physical needs. Identify functional limitations and develop appropriate therapeutic goals. Treatment Planning and Intervention Develop individualized physical therapy treatment plans based on evaluation findings and individual goals. Implement therapeutic exercises, mobility training, and physical interventions designed to improve strength, flexibility, and functional independence. Provide therapeutic activities that support improved posture, balance, coordination, and movement. Therapy and Rehabilitation Provide one-on-one physical therapy sessions tailored to individual needs. Utilize evidence-based techniques and therapeutic modalities to address physical impairments. Assist individuals in improving mobility and performing activities of daily living safely. Progress Monitoring and Documentation Monitor and evaluate individual progress toward therapy goals. Adjust treatment plans as needed to ensure optimal outcomes. Maintain accurate and timely documentation including evaluations, treatment notes, and progress reports. Education and Support Educate individuals, families, and staff on exercises, mobility strategies, and injury prevention techniques. Provide guidance on adaptive equipment, positioning, and safe movement techniques. Train staff on physical support techniques that promote safety and independence. Collaboration and Interdisciplinary Care Collaborate with physicians, occupational therapists, speech therapists, behavioral specialists, and program staff to coordinate services. Participate in interdisciplinary team meetings to discuss progress, treatment plans, and service recommendations. Support integrated care approaches that address the overall well-being of individuals served. Perform other duties as assigned. Knowledge, Skills, and Abilities Strong knowledge of physical therapy principles, rehabilitation techniques, and mobility training. Ability to assess physical functioning and develop individualized therapy plans. Strong clinical reasoning and problem-solving skills. Excellent communication and interpersonal skills. Ability to collaborate effectively with interdisciplinary teams. Compassion and commitment to improving individuals' physical independence and well-being. Ability to adapt therapeutic strategies to meet diverse individual needs. Qualifications Doctorate or Master's degree in Physical Therapy from an accredited program. Current state licensure as a Physical Therapist. Clinical experience providing physical therapy services preferred. Experience working with individuals with developmental disabilities preferred. Physical Requirements Ability to demonstrate therapeutic exercises and assist individuals with mobility and transfers. Ability to lift or support individuals as necessary during therapy sessions. Additional Requirements Must cooperate with the licensee and Department of Human Services (DHS) staff during any inspection, inquiry, or investigation. Ability to maintain accurate clinical documentation and comply with regulatory standards. Background and Employment Requirements Successful completion of a criminal background check and clearance from the Department of Human Services prior to working with individuals served. Clearance through the Central Registry of Offenders Against Individuals with Developmental Disabilities. Completion of Child Abuse Registry Information (CARI) background checks. Participation in required drug screening programs including pre-employment, random, and for-cause testing.

Required Training and Certifications Employees must successfully complete and maintain all required training in accordance with Department of Developmental Disabilities (DDD) standards, including: CPR First Aid Emergency Procedures Medication Administration (if applicable)

$29.66 - $57.40 per hour
