Physical Therapist Thayer County Health Services, Hebron, NE
POSITION SUMMARY
Full-time physical therapist position at a rural Critical Access Hospital. A physical therapist at Thayer County Health Services will plan, interpret and implement specific rehabilitation treatment programs for patients in accordance with all applicable regulatory requirements and state laws to provide high quality, professional rehab services in accordance with the values and standards set forth by Thayer County Health Services. Provides care for patients in the community within outpatient, swing bed, inpatient, home health, assisted living and long-term care settings.SUPERVISION

Minimum of Bachelor's Degree in physical therapy required; Doctorate Degree preferred. Must be designated as a Registered Physical Therapist by the State Board of Medical Registration and Examination. Experience in Physical Therapy preferred. Must hold current and valid Driver's license. Must have motor coordination, strength, and physical abilities to perform all job duties.PRINCIPAL RESPONSIBILITIES
Plans and evaluates patients' treatment programs in coordination with medical staff. Recommend changes. Administer treatment programs developed by therapists and physicians. Clarifies physician's treatment instructions. Collaborate with physicians and medical staff on patient care issues. Evaluates patient needs. Discusses condition and treatment plans with patients, family and medical staff. Evaluates charts and reports on patients' progress. Bills patients according to established . Supervises and directs assistants and other qualified staff in administering treatment, ensuring high-quality care. Actively participates in quality improvement initiatives, engaging proactively in education, service line expansion, skill development and knowledge acquisition. Recommends physical therapy policies and procedures concerning services provided. Maintains quality assurance program, safety, environmental and infection control. Recommend solutions to patient care problems. Maintains patients' records, charts, and discharge summaries. Educate patients and family in proper care and use various support and exercise programs. Assist other therapists in planning treatment programs. May conduct or participate with training medical and nursing personnel. Assists with data collection and prepares reports. Maintains department in a clean and orderly manner. Keeps equipment in good working condition. Attend required meetings and participate in committees as requested. Maintains strict patient confidentiality. Utilizes and demonstrates proficiency in use of mechanical and modality equipment, use of assistive devices and physical therapy treatment techniques Maintains requirements to keep NE physical therapy license active, current and legal.PERFORMANCE STANDARDS
Maintains and promotes good health practices. Will conform to TCHS procedures and policies. Will perform all duties as described by the job description within the scope of licensure. Maintains current professional license in accordance with state law. Does not discuss patient matters such as medical treatment, diagnosis or history outside of the clinical setting in, keeping in compliance of HIPAA. Understand and comply with the requirements of Thayer County Health Services' Compliance Program, including, but not limited to, the Code of Conduct, the Compliance Manual. All supporting policies for compliance and compliance plans. Participate in all education and training programs regarding compliance as required by organizational policy and as requested by supervisor. Demonstrate formal greetings with patients, visitors and TCHS staff. Willingly accepts new responsibilities and cooperates with implementation of change. Dresses in a neat, clean uniform for patient care maintain high personal hygiene standards. Demonstrates respect, integrity, compassion, and excellence through a "We Care" philosophy with all patients, visitors, and TCHS staff.WORKING CONDITIONS
Works in patient home environment with a variety of external factors. Works mainly in an indoor, smoke-free, climate-controlled environment. Job duties will require therapist to outdoors occasionally Requires full range of body motion including manual and finger dexterity and eye-hand coordination Potential exposure to infectious material. Frequent standing and walking for prolonged periods of time. Requires handling and lifting of patients as well as bending, lifting and stooping. Requires ability to use and maintain department equipment. Must be able to lift and carry up to 50 pounds.Similar remote jobs
