$2,000 Sign on Bonus Mileage Reimbursement Company Overview Visiting Nurse Home Care & Hospice of Carroll County & Western Maine is a dedicated non-profit organization committed to providing comprehensive in-home healthcare services. As a licensed Medicare Certified agency, we deliver skilled nursing, therapy, and hospice services to support individuals in maintaining independence and quality of life within their homes. Job Summary We are seeking a dedicated and skilled Home Health Physical Therapist to join our compassionate healthcare team. The ideal candidate will possess a strong foundation in physiotherapy, with expertise in developing care plans and delivering patient-centered therapy across diverse settings. This role involves working closely with patients and interdisciplinary teams to promote recovery, mobility, and overall well-being within the home environment. Essential Job Functions Provides physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act. This may include, but will not be limited to:
- Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluate the home environment.
- Assesses for muscle strength, mobility, gait, ROM—potential for rehab.
- Directing physical therapy treatment.
- Instructing patients and families/caregivers in the use and care of therapeutic appliances.
- Determining priority needs for physical therapy.
- Reporting to physician patient's reaction to treatment or changes in condition. Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
- May train patients in the use of prosthetic device.
- Identifies patient and family/caregiver needs for other home health services and refers as necessary.
- Prepares and submits clinical and progress summaries based on the attainment of goals.
- Participates in discharge planning for patients.
- Provides physical therapy consultation to home families/caregivers when indicated.
- Provides in-service education programs for clinical staff as needed.
- Participates in peer consultation process.
- Supervises physical therapy assistants according to organization policy and state regulations. The incumbents may be requested to perform job related tasks other than those stated in this description. Position Qualifications
- Doctorate in Physical Therapy from an approved accredited organization.
- Licensed to practice as a physical therapist within the state of Maine and New Hampshire
- Two years of applicable experience as a physical therapist. Community/home health experience is preferred.
- Demonstrates good verbal and written communication skills.
- Maintains current CPR certification.
- Possesses a current driver's license and insured vehicle Physical Demands The physical demands described here are representative of those that must be met by a team member to successfully perform the essential functions of this position.
- Ability to stand and walk for extended periods during patient treatments and facility rounds.
- Frequent bending, stooping, reaching, and manual manipulation to assist patients with exercises and mobility.
- Ability to lift, move, and support patients and equipment weighing up to 50 pounds independently, and more with assistance.
- Sufficient visual acuity to observe patient responses, read charts, and operate therapy equipment.
- Adequate hearing to respond to patient needs and communicate effectively with team members.
- Manual dexterity to operate therapeutic tools, adjust equipment, and document patient progress.
- Stamina to perform repetitive motions and maintain physical activity throughout the workday.
