Job Description
Overview Join our dynamic healthcare team as a dedicated Physical Therapist, where your expertise will transform patient recovery journeys and enhance quality of life. In this vital role, you will assess, plan, and implement personalized physiotherapy interventions across diverse outpatient clinics. Duties Conduct comprehensive assessments of patients' physical conditions, including mobility, strength, flexibility, and pain levels. Develop and implement individualized care plans tailored to each patient's needs, utilizing evidence-based physiotherapy techniques. Administer therapeutic interventions such as trigger point therapy, manual therapy, therapeutic exercises, and modalities to promote healing and function. Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless transitions across inpatient, outpatient, home health, and community care settings. Educate patients and their families on injury prevention, proper movement techniques, and self-management strategies to support long-term wellness. Maintain accurate documentation of patient progress using Electronic Medical Record (EMR) systems while adhering to healthcare regulations and medical terminology standards. Monitor patient responses to treatment plans regularly; modify approaches as needed to optimize outcomes. Assist in the management of care plans for special populations such as pediatrics and geriatrics, ensuring age-specific considerations are met. Participate in team meetings to review patient progress, discharge criteria, and ongoing care strategies. Experience Proven experience working as a Physical Therapist. Demonstrated success in outpatient clinics or private practices delivering physical therapy services across diverse patient populations. Hands-on experience with home & community care programs that involve home health visits or outpatient rehabilitation services. Strong knowledge of physiology, anatomy, medical terminology, and orthopedic principles essential for effective treatment planning. Familiarity with EMR systems for documentation and patient record management. Experience in discharge planning processes that facilitate smooth transitions from hospital to home or community settings. Knowledge of specialized therapies such as trigger point therapy or orthopedics is highly desirable.
