Responsibilities Assist in evaluating patients' physical conditions and contribute to the development of comprehensive care plans tailored to individual needs. Implement therapeutic interventions such as therapeutic exercises, manual therapy techniques including trigger point therapy, and modalities to enhance mobility and reduce pain. Support discharge planning processes by preparing patients for transition to home or community care settings, ensuring continuity of care. Provide specialized care for diverse populations including pediatrics, geriatrics, orthopedics, and patients recovering from acute care episodes. Document patient progress accurately using electronic medical records (EMR) systems and communicate updates effectively with the healthcare team. Educate patients and their families on physiotherapy techniques, injury prevention, and home exercise programs to promote long-term health benefits. Monitor patient responses during therapy sessions and adjust treatment approaches accordingly to optimize outcomes. Skills Strong knowledge of physiology, anatomy, and medical terminology essential for accurate assessment and treatment planning. Experience with inpatient hospital environments, outpatient clinics, or home & community care settings demonstrating adaptability across various healthcare contexts. Proficiency in discharge planning, care plan development, and understanding of outpatient and home health protocols. Familiarity with EMR systems for efficient documentation and communication within multidisciplinary teams. Expertise in physiotherapy techniques including trigger point therapy, orthopedics rehabilitation, geriatrics-focused interventions, and pediatric care. Ability to work effectively in fast-paced environments such as acute care hospitals or outpatient clinics while maintaining attention to detail.
