Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to provide patient-centered rehab services for seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will deliver therapeutic exercises, gait and balance training, functional mobility, and patient education to improve safety, independence, and quality of life. In our collaborative culture, you'll benefit from mentorship, manageable caseloads, and strong clinical support while contributing to meaningful functional outcomes for every patient you serve. Responsibilities Provide direct PTA treatments under the supervision of a Physical Therapist for seniors in skilled nursing and post-acute settings Implement individualized care plans focused on improving mobility, strength, balance, and functional independence Accurately document treatments, patient progress, and outcomes in the EMR per regulatory and facility standards Collaborate with PTs, nursing, and interdisciplinary teams to adjust plans of care and support discharge planning Educate patients and families on home exercise programs, safety, and strategies to maintain function Maintain compliance with federal, state, and facility policies, including infection control and safety standards Participate in case conferences, in-services, and clinical education to support continuous improvement Contribute to a respectful, patient-centered culture that prioritizes dignity, compassion, and quality outcomes Required Skills Therapeutic exercise implementation Gait and balance training Neuromuscular re-education Functional mobility training Pain management modalities Patient and family education Documentation in EMR systems Knowledge of SNF/post-acute regulations Care plan implementation Infection control and safety
