About Us:
Wellness Cafe PT is a leading private practice dedicated to enhancing the lives of seniors in assisted living communities. Our mission is to support residents in maintaining independence, mobility, and quality of life. We utilize Medicare and supplemental insurance, and private pay. We are seeking a knowledgeable, dependable, and motivated Physical Therapist Assistant to lead therapy interventions for residents transitioning into our community. This role offers the opportunity to work closely with ownership.
Your Role:
As a Physical Therapist Assistant at Wellness Café PT, you will:
Team Coordination:
Assist a Physical Therapists and and aide to ensure all scheduled treatments are completed each day.
Community Exercise Classes:
Lead group exercise sessions designed to improve wellness and mobility for residents.
Therapy Plan Management:
Monitor and follow up on therapy plans of care (POC) for both new and existing residents to support their functional mobility goals.
Facility Use:
Operate from residents rooms and from the two rented rooms within the community—one dedicated to administrative tasks, the other serving as a compact gym space.
Environmental Assessments:
Evaluate both home and communal living environments to enhance safety, encourage independence, and prevent falls.
Therapy Delivery:
Provide targeted interventions including balance training, gait enhancement, posture correction, strength development, and long-term maintenance programs. Conduct Home evaluations in the community for new residents: to assure their safety.
Efficient Documentation:
Complete accurate and timely records using WebPT, while researching potential EMR alternatives for workflow improvements.
Community Engagement:
Facilitate educational and exercise-focused classes that empower residents to stay active and informed.
