The position is based in a skilled nursing and outpatient rehabilitation environment that serves a diverse adult and geriatric population. The facility is part of a larger healthcare campus that focuses on providing high-quality rehabilitative care, short-term rehab, and long-term support. Patients include individuals recovering from orthopedic surgeries, neurological events, debility, and other conditions requiring skilled therapy services. The environment emphasizes collaboration, with therapy staff working closely alongside nursing, medical providers, and support staff to create comprehensive, patient-centered treatment plans. This role features a 5x8 schedule with 40 hours per week guaranteed, providing consistent, full-time work and predictable days. The caseload is approximately 60 percent skilled nursing facility and 40 percent outpatient, offering a dynamic mix of higher-acuity post-acute care and more traditional outpatient therapy. This combination allows you to maintain and grow skills across multiple care levels, from functional mobility and balance training to strengthening, pain management, and return-to-activity programs. On a typical day, you will provide direct patient care under the supervision of a licensed Physical Therapist, implementing individualized treatment plans for residents in the SNF as well as patients scheduled in the outpatient clinic. In the SNF portion of your day, you can expect to assist with bed mobility, transfer training, gait training with assistive devices, fall-prevention programs, and functional activities of daily living to promote safe discharge back to home or community settings. You will work with an interdisciplinary team to support rehabilitation after joint replacements, fractures, strokes, and other complex medical conditions, contributing to weekly or biweekly care conferences. In the outpatient setting, you will help treat patients with orthopedic, post-surgical, and chronic musculoskeletal conditions. Responsibilities may include assisting with therapeutic exercises and neuromuscular re-education, supporting manual therapy programs as directed by the PT, setting up and monitoring modalities, and guiding patients through progressive strengthening and functional programs tailored to their goals. The outpatient environment provides the opportunity to see patients progress over time, foster strong therapeutic relationships, and refine skills in exercise progression and patient education. Patient load and scheduling are structured to support safe, high-quality care while maintaining productivity expectations. You can expect to manage a steady caseload, with the balance of SNF and outpatient allowing variety in diagnoses, functional levels, and treatment approaches. The facility emphasizes teamwork and communication, providing access to other rehabilitation professionals and nursing staff for support.
