Physical Therapists are responsible for:
the assessment and treatment of patients; completing documentation; planning and developing new programs; communicating with patients, patients' families and physicians and staff; presenting in-services; and assisting in general operation of the department.
Key Responsibilities:
Perform comprehensive evaluations of residents to assess strength, mobility, balance, coordination, and functional ability. Develop and implement personalized physical therapy treatment plans based on clinical findings and physician orders. Provide therapeutic interventions including exercise, gait training, manual therapy, pain management, and use of modalities Document all evaluations, treatments, and resident progress in accordance with regulatory, facility, and professional standards. Collaborate with nurses, physicians, occupational therapists, speech therapists, and other members of the healthcare team to ensure coordinated care. Comply with all federal, state, and facility policies including HIPAA, Medicare/Medicaid documentation guidelines, and infection control protocols.
Requirements:
Must be a graduate of a accredited therapy program (Bachelor Degree required. Masters Degree Preferred) Must possess or be eligible for state licensure Some previous inpatient rehab or LTC/SNF experience preferred but not required.
