Job Overview We are seeking a dedicated and skilled Physical Therapist to join our compassionate healthcare team. The ideal candidate will possess a strong background in physical therapy, with experience across inpatient, outpatient, and home health settings. As a Physical Therapist, you will be responsible for developing personalized care plans, delivering patient-centered therapy, and contributing to the overall well-being of our residents. Outpatient & Skilled Nursing What We're Looking For Doctor of Physical Therapy (DPT) degree Current state PT license (or eligibility) Strong communication and patient engagement skills A positive, motivated mindset with compassion at the core New grads welcome—we love developing talent! Duties Assess patients' physical conditions by evaluating mobility, strength, and functional abilities using comprehensive physiotherapy techniques. Develop and implement individualized care plans that incorporate evidence-based interventions tailored to each patient's needs. Administer treatments such as trigger point therapy, orthopedics-focused therapy, and geriatrics rehabilitation to optimize recovery outcomes. Coordinate discharge planning with interdisciplinary teams to ensure seamless transitions from hospital or inpatient settings to home or community care. Maintain accurate documentation of patient progress while adhering to confidentiality standards. Educate patients and their families on physiotherapy exercises, injury prevention, and health maintenance strategies. Collaborate with medical professionals to interpret medical terminology, physiology knowledge, and anatomy understanding for effective treatment planning. Participate in ongoing professional development activities to stay current with advancements in physiotherapy practices and healthcare regulations. Experience Proven experience working in acute care hospitals, outpatient clinics, Long-term Care, or home health environments with a focus on patient care delivery. Demonstrated expertise in discharge planning, care plan development, and implementing physiotherapy techniques such as trigger point therapy. Experience working with diverse populations including pediatrics, geriatrics, and orthopedic patients within inpatient or outpatient settings. Knowledge of medical terminology, anatomy, physiology, and the principles of physiotherapy essential for effective practice. Prior involvement in home & community care programs that emphasize patient independence and recovery support. Strong communication skills to effectively collaborate with multidisciplinary teams and educate patients about their treatment options.
