Full Job Description Physical Therapist Needed:

Home Care in Central Suffolk County New York Therapy and Wellness is expanding our footprint on Central Suffolk County. Founded by a husband-and-wife clinical team, we are looking for a Physical Therapist who wants to escape the burnout of high-volume clinics or the isolation of typical home care. Responsibilities:

Treat geriatric and/or pediatric patients in their homes within Central Suffolk County. Create and execute personalized care plans that debunk the myth that home care is "low skill." Submit clinical documentation in a timely manner. Communicate openly with the leadership team—we want to hear your ideas and feedback.

Qualifications:

License:

Valid NY State Physical Therapy License (Required).

Education:

Bachelor's or Doctorate degree in Physical Therapy.

Experience:

New Grads Welcome! We invest in training the right people.

Transport:

Ability to travel to patient homes in Central Suffolk County.
