The ideal candidate will possess a strong background in patient care, with experience in various settings including inpatient, outpatient, and home health. This role involves developing and implementing effective treatment plans tailored to individual patient needs, ensuring optimal recovery and rehabilitation. Responsibilities Conduct comprehensive evaluations of patients to assess their physical abilities and limitations. Develop personalized care plans that address the specific needs of each patient, utilizing evidence-based practices. Implement therapeutic interventions such as physiotherapy techniques, trigger point therapy, and orthopedic rehabilitation. Collaborate with multidisciplinary teams to coordinate discharge planning and ongoing patient care. Educate patients and their families on treatment protocols, exercises, and strategies for managing their conditions at home. Maintain accurate documentation of patient progress using EMR systems, ensuring compliance with medical terminology standards. Provide care across various settings including acute care hospitals, outpatient clinics, geriatrics, and home & community care environments. Stay updated on advancements in physiotherapy practices and participate in professional development opportunities. Requirements Valid physical therapy license in the state of practice. Strong knowledge of anatomy and physiology relevant to physical therapy practices. Familiarity with discharge planning processes and the ability to create effective care plans for diverse populations. Previous experience working with geriatric patients is a plus. Proficiency in using EMR systems for documentation purposes. Excellent communication skills to effectively interact with patients, families, and healthcare professionals. Ability to work collaboratively within a team-oriented environment while maintaining a patient-centered focus.
