Job Description

Overview Join our dynamic healthcare team as a Physical Therapist, Occupational Therapist, or Physical/Occupational Therapy Assistant, where your expertise will directly impact patient recovery and quality of life. In this role, you will deliver comprehensive therapy services across inpatient, outpatient, home health, and community settings. Responsibilities Assess patients' physical and functional conditions to develop personalized care plans aligned with their recovery goals. Implement therapeutic interventions including physiotherapy techniques, trigger point therapy, orthopedics, geriatrics, pediatrics, and community care strategies. Monitor patient progress regularly and adjust treatment approaches to optimize outcomes. Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless transitions between inpatient, outpatient, home health, and community services. Educate patients and caregivers on exercises, safety measures, and home modifications to support ongoing recovery. Document all patient interactions accurately within electronic medical record (EMR) systems while maintaining compliance with healthcare regulations. Participate in hospital or clinic-based care activities such as acute care management, care plan development, and patient education sessions. Qualifications Valid licensure or certification as a Physical Therapist, Occupational Therapist, or Therapy Assistant in the relevant jurisdiction. Demonstrated experience in inpatient hospital settings or outpatient clinics with a focus on physiotherapy or occupational therapy. Strong knowledge of anatomy, physiology, medical terminology, and rehabilitation techniques including trigger point therapy and orthopedics. Experience working with diverse populations such as geriatrics and pediatrics in hospital or home & community care environments. Familiarity with EMR systems for documentation and care coordination purposes. Ability to perform discharge planning effectively while managing acute care cases with attention to patient safety and mobility needs. Excellent communication skills to collaborate with healthcare teams and educate patients on treatment plans.
