Responsibilities:

Evaluate and treat patients to restore, maintain, and improve physical function and mobility Develop individualized treatment plans and track progress according to facility standards Collaborate with the interdisciplinary team to ensure high-quality patient care Deliver compassionate, patient-centered services

Requirements:

Graduate of an accredited Physical Therapy program Active Ohio PT license Strong clinical reasoning, documentation, and communication skills Previous SNF experience preferred but not required
