Full-time Monday-Friday + occasional weekends (as needed)
Location:
Field Based Compensation (Productivity Points): $75.00 / point | $100,000 - $130,000 /year
Who We Are:
Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our mission is to dramatically improve patient access to home health care . The PT role is a core part of that mission: you restore function, keep patients safe at home, and drive real outcomes. We also build technology to support field clinicians. Our charting product reduces documentation burden, and our scheduling product (launching soon) will make routing and visit coordination easier — protecting your time.
What you will do:
Own the caseload as a case manager: you're accountable for the patient's therapy plan, progression, and outcomes — not just individual visits. Complete comprehensive in-home assessments and deliver skilled PT visits. Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention. Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.
Drive multidisciplinary coordination:
communicate changes in condition, align on orders, and keep the care team moving. Manage treatment and outcomes directly, including PTA supervision where appropriate. Coach patients and caregivers with practical home programs that actually get done. Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent. Requirements Must-haves Active Texas PT license (in good standing) Strong clinical judgment and comfort working independently in the field Comfortable escalating changes in condition to RN/MD and documenting rationale clearly Strong communication and documentation Reliable follow-through on orders, frequencies, and patient goals Current driver's license and comfort with daily travel across assigned territory Able to carry/transport basic therapy equipment as needed Competency in core home health PT scope Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations Strengthening, functional mobility, and home safety assessment/modification education Patient/caregiver coaching with practical home programs Nice-to-haves Prior home health experience Homecare Homebase (HCHB) experience (or similar home health EMR) Deeper clinical experience in high-signal PT populations Experience supervising/mentoring PTAs and collaborating on care plans Strong organization and time management
