Per Visit / Per Diem / Flexible Assignments Job Overview We are currently seeking an experienced and compassionate Registered Nurse (RN) for Home Health assignments in Houston, Texas. The ideal candidate must have prior Home Health experience and be proficient in using the Axxess EMR portal for documentation and patient care coordination. This role is perfect for nurses who are dependable, detail-oriented, and passionate about delivering high-quality patient care in a home setting. Responsibilities Conduct skilled nursing visits in patients' homes Complete accurate and timely documentation using the Axxess portal Perform patient assessments and develop care plans Administer medications and treatments as ordered Educate patients and families on health management Coordinate care with physicians, therapists, and interdisciplinary teams Monitor patient progress and report changes in condition Requirements Active Texas RN License Minimum 1 year of Home Health experience preferred Experience using Axxess Home Health software

REQUIRED
