Primary Responsibilities:

Perform in-home physical therapy evaluations and mobility assessments Create individualized treatment plans centered on safety, strength, and functional progress Deliver hands-on therapy, gait training, and home exercise instruction Recommend equipment and home modifications to enhance independence Collaborate with nurses, caregivers, and interdisciplinary providers

Requirements:

Active Minnesota Physical Therapist license Valid driver's license and dependable transportation Home health experience a plus, but not required—outpatient and inpatient PTs encouraged to apply Passion for empowering patients through one-on-one, goal-driven care
