Physical Therapist - Travel Contract

Minimum Requirements Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Active Illinois Physical Therapist license required to start the assignment. BLS certification from AHA or ARC may be required. Experience working in a Skilled Nursing Facility or long-term care setting preferred. Familiarity with documentation systems common to SNFs, including electronic medical records and Medicare-required forms, preferred. Experience with skilled therapy services for geriatric populations, including ADL retraining, fall prevention, and restorative programs, preferred.
