Purpose:

The purpose of this position is to provide exceptional student (special education) services by filling the vacant contract positions in Physical Therapy and Physical Therapy Assistance at Crownpoint Community School (CCS) for the fiscal school year 2026-2027.

Scope:

Physical therapy in person services will be delivered in a culturally competent manner. The specific methodology incorporates a in person-centered approach which will provide empowerment to CCS students. The primary purpose is to focus on building effective relationships with students, educators, administrators, and parents of CSS. The service providers will facilitate and support each CCS student to reach their highest educational potential in becoming productive members of a multicultural society. Duties An licensed Physical Therapist (PT) and assistant (PTA) are to provide occupational services to identified students in the CCS Special Education department. The physical therapist and/or physical therapist assistant will work onsite one day per week, throughout the fiscal school year, to ensure these identified students receive adequate PT services as mandated on their individualized education plans. An PT/PTA will evaluate and interpret evaluation results for the IEP committee. PT/PTA interventions will be designed and implemented appropriately to meet the needs of the individual students as determined by the IEP committee. The PT/PTA will provide teacher consultation services. The PT/PTA will provide daily performance data, assist with goal writing, participate in IEP meetings for students on caseload, keep an active roster of students, submit case logs, and submit progress reports promptly, provide evidence of services through documentation of service log completion at the end of each session to be filed appropriately in NASIS. Conduct physical therapy to identified students. Conduct comprehensive occupational, physical therapy evaluations to assist the exceptional education department in whether a child meets or still meets the requirements for an exceptional educational placement. The assessment toolsto be used are standardized and norm-referenced. Review and analyze current evaluation reports Review student medical and educational records Evaluations are required to be completed within 60 days as mandated by IDEA. The service provider contractors will submit quarterly progress notes and therapy logs for students promptly. The service provider contractors will sign in and sign out verifying attendance at school. All Service providers are to provide their own personal equipment (laptops, kajeets, printers, supplies/materials, curriculum materials)

Pay:

$35.94 - $43.28 per hour
