Job Description

Join University of Rochester Medicine Home Healthcare and be part of a team dedicated to delivering exceptional, patient-centered care in the comfort of patients' homes. As a PTA, you'll work one-on-one with patients, helping them regain strength, mobility, and confidence while collaborating closely with a supportive interdisciplinary team.
Schedule:
Monday - Friday, 8 to 430pm, no weekends, no holidays, no on call
Location:
Serving Ontario & Yates County Minimum Qualifications:
Graduate from an accredited school of physical therapy assistants License to practice as a physical therapy assistant in the state of New York Two (2) years of physical therapy assistant experience Current CPR certification NYS driver's license Reliable transportation and Agency required liability insurance
Summary:
Assists Physical Therapy in providing services to clients in their homes to improve and/or restore functionality of neuron-muscular, skeletal, pulmonary and/or cardiovascular disorders. Confers and collaborates with the physical therapist to implement the home care treatment plan. Independently provides and documents visits, to clients across the age span with a variety of needs. Collaborates with other members of the client care team.
RESPONSIBILITIES
Works under the supervision of a licensed physical therapist performing such patient related activities as are assigned by the supervising physical therapist. Duties of physical therapist assistants shall not include evaluation, testing, interpretation, planning or modification of patient programs. Administer, but not develop, treatment plans created by Physical Therapist Monitor and document patient progress/changes and report any changes in patient status to the PT. and case manager in timely and accurate manner. Initiates a systematic, accurate and ongoing review of biophysical, psychosocial, environmental, self-care, safety, educational, and discharge planning needs of the client, family, and/or significant other. Provides home care visits as assigned and follows through with each plan of care based on identified client needs. Makes recommendations to Physical Therapist for adaptive equipment needs and arranges for necessary adaptive equipment as needed as assessed and deemed appropriate by Physical Therapist Assures completion of all appropriate documentation needed for compliance with state and federal legislation and the delivery of seamless service within established timeframes Provides home care visits as assigned while completing each week's planning for scheduled visits, communicating changes in daily scheduling as they occur with appropriate supervisor and/or covering clinician to ensure 24/7 coverage for client care. Follows the plan of care developed by the Physical Therapist and communicates timely any changes needed to achieve goals. Communicate results and recommendations to clients and other care providers based on the client's needs. Reports on progress and changes in the client's condition to case manager. Collaborates with client, family and interdisciplinary team members in working towards goals. Demonstrates knowledge, skills, and abilities necessary to provide care to the age groups served. Demonstrates an understanding of accreditation, state & federal regulations governing home health agencies and maintains compliance with regulations/organizational policies and procedures. Maintain productivity and quality requirements.
Salary Range:
$17.46 - $34.57 The referenced pay range represents the minimum and maximum compensation for this job family. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations. #URM1
