Job description Community Care Internal Medicine - Delmar, part of Community Care Physicians (CCP), is a well-established primary care practice focused on delivering high-quality, evidence-based care to adult patients. The practice features an experienced team of board-certified internists and advanced practitioners dedicated to patient-centered care in a comfortable and accessible environment. Located within the Delaware Avenue Health Park, the practice offers convenient access to a full range of specialty and ancillary services, supporting coordinated, comprehensive care for patients. CCP is a physician-led, multispecialty medical group with a long-standing commitment to improving community health through collaboration, innovation, and high-quality care. We are seeking a full-time Physician to join our adult primary care team. This position is open to Internal Medicine or Family Medicine physicians who are interested in caring for patients age 18 and older in a collaborative, team-based environment. The ideal candidate is committed to delivering compassionate, evidence-based care, building long-term patient relationships, and contributing to quality outcomes. Key Responsibilities Provide comprehensive primary care services to adult patients (18+). Diagnose and manage acute and chronic medical conditions. Perform preventive care, annual physicals, and age-appropriate screenings. Offer timely access to care, including same-day and urgent visits when needed. Collaborate with advanced practitioners, nursing staff, and specialists to ensure coordinated care. Utilize electronic health records and telehealth tools to enhance patient access and continuity. Participate in quality improvement initiatives and practice development activities. Educate patients on disease prevention, health maintenance, and treatment plans.
