Job Description

Responsibilities:
  • Provide primary care services to patients, including physical examinations, diagnosing and treating common illnesses and injuries, and prescribing medications. Provide Urgent Care evaluation, Suturing or Laceration repair.
  • Collaborate with physicians and other healthcare professionals to develop and implement patient care plans.
  • Perform procedures such as suturing, wound care, and gastric lavage.
  • Order and interpret diagnostic tests, such as medical imaging and laboratory tests.
  • Provide patient education on disease prevention, treatment options, and healthy lifestyle choices.
  • Assist in discharge planning and coordination of care for patients transitioning from hospital to home or other healthcare facilities.
  • Document patient encounters using electronic health record systems such as Athena or Cerner or eClinicalWorks.
  • Participate in quality improvement initiatives to enhance patient outcomes and optimize healthcare delivery.
Qualifications:
  • Valid Nurse Practitioner OR Physician Assistant license in the state of practice.
  • Master's degree in Nursing with specialization as a Nurse Practitioner.
  • Strong clinical knowledge and expertise in primary care.
  • Excellent communication and interpersonal skills to effectively collaborate with patients, families, and healthcare team members.
  • Proficient in medical coding and documentation practices.
  • Experience in home care or long-term care settings is preferred.
  • Ability to effectively manage data and utilize technology for data management purposes.
