Job Description
Job Overview We are seeking a dynamic and compassionate Primary Care Nurse Practitioner to join our healthcare team. In this vital role, you will provide comprehensive primary care services, promote health and wellness, and manage a diverse patient population across various age groups. Your expertise will help improve patient outcomes through thorough assessments, personalized treatment plans, and collaborative care. Duties Conduct detailed patient assessments including vital signs, physical examinations, and medical histories to determine health status. Develop and implement individualized treatment plans, including medication management, preventive care, and health education tailored to each patient's needs. Utilize the electronic health record (EHR) system, eClinicalWorks, to document patient encounters accurately and efficiently. Collaborate with multidisciplinary teams on complex cases including vascular/ arterial cases, behavioral health, podiatric medicine, acute pain management, etc. Provide telehealth consultations to expand access to care and ensure continuity for remote or underserved patients. Educate patients about disease prevention, medication administration, nutrition, and lifestyle modifications to promote overall wellness. Qualifications Valid Nurse Practitioner license with certification in family practice or primary care. Must be fluent in Spanish. Proven experience in medical office settings with familiarity in EMR/EHR systems. Strong clinical skills including phlebotomy, injection administration, and sterile technique. Experience working with diverse populations including children, seniors, individuals with disabilities or developmental disabilities, and those requiring hospice or palliative care. Knowledge of CPT coding for billing purposes and ICD coding (ICD-10/ICD-9) for diagnostic evaluation. Critical care experience preferred; familiarity with hospital medicine, ICU or emergency room settings is advantageous. Ability to perform diagnostic evaluations. Excellent communication skills for triage assessments and patient education. Knowledge of HIPAA regulations ensuring patient confidentiality and data security.
