This group is experiencing significant growth and offers a value-based care model focused on quality outcomes, smaller patient panels, and meaningful provider-patient relationships. Physicians are supported by a collaborative, team-based environment that includes dedicated clinical and administrative resources, allowing you to practice medicine with a strong emphasis on preventative care and population health management.

Responsibilities:

Provide primary care to a panel of senior patients with a value-based care organization focused on Medicare Advantage. 36 patient-facing hours per week Average 12-14 patients/day Monday

• Friday; 8am
• 5pm No weekends and limited call rotation for telephonic engagement only

Qualifications:

Must be board certified Internal Medicine or Family Medicine with experience in an outpatient setting. DEA with no restrictions.
