00 per hour, depending on experience PRN / as-needed scheduling Service AreaSchedule Cleveland / Northeast Ohio | Flexible scheduling available. What You'll Do Provide skilled nursing care to hospice patients in their homes in accordance with the established plan of care Conduct assessments and monitor patient symptoms, comfort levels, and changes in condition Administer medications and treatments focused on comfort and quality of life Serve as a key point of contact for patients, families, and the interdisciplinary care team Provide education and emotional support to patients and families navigating end-of-life care Complete timely and accurate documentation in the EMR within required timeframes Participate in on-call responsibilities as scheduled What Sets VNA Apart AI-assisted documentation

Requirements Must-Have:
Active RN license (Ohio) + current BLS certification Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area Minimum 1 year of clinical nursing experience Preferred /
Nice-to-Have:
Prior hospice experience Experience with end-of-life care and comfort-focused care delivery Familiarity with HCHB or similar home health/hospice EMR Strong communication and family support skills
